Do I need a referral from my doctor?
Eventually, yes. New York State requires a referral from an authorized health care provider after the 10th physical therapy visit or 30 days, whichever comes first.
Authorized heath care providers include: physicians, nurse practitioners, podiatrists, dentists or midwives — licensed and currently registered to practice in New York State.
Rate information / How does sliding scale work?
Jael’s rate is sliding scale $75-125 per session. The sliding scale payment system allows you to determine a rate within the range that works for you.
Jael accepts Venmo (@jaelevator), cash, or check payment.
Can I use my insurance?
If your insurance plan includes out-of-network physical therapy coverage, you may be able to use your insurance to contribute to physical therapy cost. Bring your insurance card to your first visit and Jael can look into your insurance eligibility and benefits.
What should I wear?
Loose fitting shorts, t-shirts, and tank tops are all good options. The work Jael does often requires direct contact with the skin — loose clothes typically allow for better access than pants and leggings. Ultimately, your comfort is most important so if you have any issues with these clothing options, please wear what makes you feel most comfortable.
What happens if I need to cancel or change an appointment?
Please reschedule or cancel at least 24 hours before your appointment time. Any cancellation with less than 24 hours notice will result in a cancellation fee of $40. No-shows will result in a fee of $75. Please keep the cancellation policy in mind as you book an appointment.

What happens if I am running late?
Please call, text, or email Jael if you are running late for your scheduled appointment time. Being late to an appointment may result in modified treatment charged at full price.
How often should I come in?
We will determine frequency together at the first session. Type and severity of injury, schedule, and budget will all factor into how often you will come in. Jael has clients that come in weekly, biweekly, monthly, quarterly, and as needed. We’ll make it work for you.
Is the location ADA accessible?
Unfortunately no. There are several steps to enter the building and a flight of stairs inside with no elevator access. If you would like recommendations for clinics that are ADA accessible, Jael would be more than happy to help you find a physical therapist that meets your needs.
What are Jael’s house rules?
Jael practices in a dedicated closed door office within her home. There are often other people in the house, but your session will always be in her private office.
